During your first visit with us, we would like to get to know you better and therefore take special time to discuss your wishes and concerns with you. To save you time on site, you are welcome to fill out our medical history form in advance and bring it with you to your appointment - alternatively, send us the form in advance by e-mail (info@herzozahn.de). Please also remember medication lists, your allergy passport and your insurance card.
Our entire practice is barrier-free, with a lift leading up to the first floor.
